Spanish Biliterate Community Organizer

Health Action Together: A Project of Community Partners
Time-Limited
Community Organizer
(Spanish Biliterate)*

Organization and Initiative Background:

The vision of Health Action Together is: A Sonoma County where a person’s skin color, zip code, or place of birth does not determine their health, education, or income. This work is guided by our values: equity at the center, upstream focus, community-led, integration and collaboration, and shared outcomes. Our work focuses on addressing inequities in our community by driving policy, institutional, and resource and investment changes and creating a model of change where community members most impacted by inequalities have a say in and lead policy, investment, and change efforts. The latest Portrait of Sonoma County Update 2021 revealed the many persistent and growing inequities in the health, education, and income of our community. Health Action Together is evolving its approach and establishing itself as an anti-racist community-led non-profit effort to help meet this need.
