Community-Led Call To Action

The Agenda for Action is here!  Thank you to all of our partners for supporting this work, and most importantly, thank you to the community for your dedication, commitment, and love for this process.

The Agenda for Action (AFA) is a call to action for leadership throughout Sonoma County to come together to invest sustainably in protecting and supporting the wellbeing of the community, and specifically the community members who have been most impacted by an interlocking ecosystem of systemic inequities. 

The AFA aims to mobilize multi- stakeholder collaboration among governments, funders, public and private sectors, and other community partners to transform access to health, education, and economic opportunities and to ensure people across Sonoma County have the resources to thrive and reach their full potential.

Health Action Together will begin by developing an Agenda for Action in response to the Portrait of Sonoma 2021 Update in partnership with communities most impacted by inequities as our roadmap for change. We will then align partners and communities in cross-sector change efforts focused on system interventions and grassroots and local action to activate the Agenda for Action.

Health Action Together will lead the development of a shared Agenda as the roadmap for change. The initial work of Health Action Together will be the creation of an Agenda for Action informed by the Portrait of Sonoma County 2021 Update findings and with guidance from communities most impacted by inequities. Health Action Together is developing an equity-centered process where trusted community leaders work with affected communities to outline goals, priorities, and actions to reduce inequities.

The Agenda for Action will serve as a roadmap to direct institutional, political, philanthropic, and community-based efforts and resources. The Agenda for Action will also guide the work of Health Action Together. The Agenda for Action development will be guided by communities experiencing the most significant inequities, specifically:

  • Census tracts with the lowest Human Development Index scores in the Portrait of Sonoma County 2021 Update

  • BIPOC under-resourced communities that are not represented in the data:

  • Black/AA Community

  • BIPOC Disability Community

  • AI/AN Community

  • BIPOC LGBTQ+ Community

  • Indigenous Language Communities

  • BIPOC Single mothers

  • Undocumented Community/ Farmworkers

  • BIPOC Elders & Youth

Did you know the AFA process is a place-based intervention?

Through initiatives like the Agenda for Action, Health Action Together is actively implementing place-based interventions. Place-based initiatives aim to empower and restore communities that have long suffered from systemic divestment and neglect.  We strive to center and uplift communities on the margins, starting with meeting them where they’re at. Place-based data drives the AFA and is necessary to achieve our aspiration of a Sonoma County where a person’s zip code does not determine their health, education, or income.
